PAN/FMX is a requirement to be an established patient and will fall under the 15% discount which is necessary for proper diagnosis and is recommended every 3-5 years. This discount applies to services only. Products are not included. Services for injuries/accidents that are considered by Workers Compensation or Employers LiabilIty Laws are not covered. Cannot be used with any insurance, any other offers or prescription drugs. Payment is due at time of service for any service not covered at 100%.
